Everyone failed to notice the lion fixated on the woman and then she put herself in a subordinate position to the lion. In addition, the "trainer" rewarded the lion by petting him, after the attack (re-enforcing this behavior).
 The stupid woman was displaying an insecure submission mind set and the lion was just showing dominance. The trainer would be at fault for not informing her of how she should present herself to the lion.
