Alright, this is kinda gross but we have been told the cow lives a completely normal life with the exception of this large hole in his side.

Fistulated Cow: The primary reason to have a cow with a fistula (hole) is to have access to the stomach contents. Cows do not digest their food. Small organisms that live in their stomach digest their for for them. If a cow gets sick and these organisms die, the cow will starve even if she eats. Vets take the stomach contents of healthy cows which contain these living organisms and put them into a sick cow's stomach. Hopefully, the live organisms will live and restart the digestive system of the sick

What you see is a cannulas.: They are used for research experiments. It alows you to take samples from the rumen. You can also set up one for the intestine. The only gross thing about them is when the cow has bloat and you take the plug out, all those juices come flying out.
