  clearly a lazy crane operator. No outrigger pads on the driveway, caused the crane to dig in and lose balance.

  This is what happens when a crane operator gets greedy. I'm a crane operator, I'm sitting in a 130 ton tadano right now, and I can tell you exactly what happened. The boss didn't want to rent a bigger crane, so he got Joe Blow crane service to come out with a half ass operator, who didn't properly set up the Load moment indicator. That crane has a computer that tells the operator when he is at max capacity. He either bypassed it, or set the computer wrong. Either way, he's a moron!

  This seems to be a common occurrence. You would think there would be some sort of device that could measure the force on the front of the vehicle. When it approaches zero (and is about to tip), a loud alarm goes off telling the operator to swing back. Probably for the best though, that looks like perhaps it's a hot tub time machine, can't be having people mess with the time continuum
