I don't think it actually hits her on the head. Look...she bows her head slightly before it impacts the ground yes, but her head would have traveled downwards alot faster if that had hit her. No debris is visible behind them. I think she heard it and ducked. I also think that if it had hit them they would be dead.

This was actually the result of a bitter divorce case. Which was taped in order for the killers to collect their money. The woman on the right was actually the brains behind this whole operation. Later this tape was leaked to the media when the killers asked for more money and she was convicted.
