     I'm always afraid this is going to happen to me. We have locks installed, but liftgate trailers can't be locked in, and drivers tend to be careless about chalking.

     We have a procedure at my Spartan where all docking must be clamped and turned off. Period!!!! Big safety must! Looks like there's a lot of safety violations there

    i used to work at a huge kroger warehouse..tons of trucks going in and out...used the light system and wheel chalks and dock locks..this still happened a couple times while i was there...once guy had an entire pallet of 12 packs and cat food and can goods....was a glorious mess...few times guys were in the trucks and went for rides around the parking lot.

    i guarantee he wasn't trying to drive away. and the forklift probably weight 8-9 thousand lbs. He didn't expect to start getting pushed forward so he probably jumped on the breaks after it was too late.

     Whats worse is I know that's a home depot receiving dept. The delivery driver didn't drive off. He forgot to chalk the wheels. That's what happens if you don't. It's on him. Spent too much time working at the Big Orange Box!!!!

      @jeg265no the driver did drive off, if you watch, you will see the brake lights on semi illuminate. That is why all home depot warehouses and facilities use either a dock lock, or a red light green light system...BTW i was an operations/safety supervisor for a company that did third party logistics for the home depot, i remember investigating this accident, and the driver did not comunicate with the facility that he was pulling out.

      @Gourry333its because they dont want any part of that mess. maybe that truck was left in neutral and the fork lift pushed it. thats too much science for these box movers to wrap their heads around.

      @Gourry333Hey GoGo, loading docks are built at a grade downwards towards the loading dock from street level. In fact, the rear end of that trailer is guarantied to be 4' below street level with that design of dock. Also taking into account the fact that the fork lift weighs a ton, and the truck and trailer weigh over 20 tons, trust me that truck isn't rolling forward. Not to mention the fact that the break lights came on to stop the truck after it started to pull away from the dock. There was a driver inside the

     That is what happens when you dont chock the trailer wheels and drive a 6000 pound forklift into it!

      @SemisterThe brake lights came on becuase instinct made him hit the pedal. The whells should have definitely been chalked, since the warehouse is too cheap to install docklocks. and, the fork weighs about 9000 lbs, three times what a car weighs. The driver should have been asked to step out of his truck and wait on the dock until the loader gave him an "all-clear". The loader is lucky he isn't dead. Or, did he?

    This happened to me at futureshop once. A skid with 7 tvs almost got smashed, and some employees were inside the trailer. My manager had a couple words exchanged with the driver.
